In section C, you will need to list what happened that lead to dismissal and the resolution the mitigating circumstance- on your prior form you provided reasoning for recent issues, however, the GPA issues started before these incidents with Sept 23 & Nov 23 coursework. Please speak to how these issues have been addressed/resolved.
It looks like a stronger section D is needed as well. While you provided a resolution the course repeat issue will also need to be included. Please provide a strong academic plan as to how you are going to pass your final attempt and specifically how you will raise your GPA and pace issues.

In September my daughter was in a bad car accident. I had to take care of her until she was able to take care of herself because she had a lot of broken bones. When I started classes in November, I thought I was going to be able to handle it, but I was not ready. Around January I got sick and was rushed to the ER. I ended up having to stay for a couple of weeks because my blood pressure had gotten too high. They would not release me until I got my pressure down. It was very uncomfortable being in the hospital for that long not being able to maneuver the way I wanted. In March my mom got sick, and I had to take her to the hospital. I am the only living daughter, so my brothers left me to handle everything. After being in the hospital with my mom for weeks she was released in good health.

I have been put on blood pressure medication and I have been taking it as prescribed. My mother has 24-hour care with a home health care nurse, so the load is not all on me anymore. The changes that I would generate to enable me to resolve the situations that prompted my dismissal include brainstorming my assignments ahead so I will be prepared for the upcoming. I would take this dismissal as a chance to consider my choices hereafter. Besides, I would ponder working with an academic instructor. The academic instructor will work one-on-one with me to estimate possible problem fields and formulate a scheme that incorporates a sequence of lessons and approach-based studying to tackle my educational wants.
