A physician in your agency stops by your office and asks you to meet with their 1PM patient. She provides you the following information about Peter:
Peter is a 47-year-old Latino male who is married and currently living with his wife (45) and 2 children (Samuel, M/12 and Anne, F/14). He has been a computer engineer at a large company for 15 years and is a respected colleague at the company. Peter scheduled today’s appointment to discuss his frequent stomach aches, headaches and poor sleep over the last three months.
After meeting with Peter, you learn: Peter has a conflicted relationship with his wife and they argue over parenting issues. Recently, Peter’s mother, who is 75-years-old, is diagnosed with liver cancer. He has a fairly close relationship with his mother who lives two hours away from Peter. Peter does not have an active social life and he spends most of his time either at work or with his family.
Using the Live, Work, Play, Health Habits framework of the Contextual Interview please provide/discuss the following:
Provide examples of questions and dialogue that you asked Peter when trying to assess each domain of the Contextual Interview (Live, Work, Play & Health Habits). You have complete creative freedom to create Peter’s responses.

BHC: What do you enjoy doing in your free time?
Peter: I’m not really sure I know how to answer that. Between my work and my mom’s cancer diagnosis, I’m not sure I really do anything for fun or that brings me joy.
As we’ve discussed in previous weeks, sometimes the use of a structured screening tool can be beneficial to identify current severity of mood and track mood over time (we’ll continue to learn more about this next week). Based on Peter’s responses to your questions and his presenting problems, do you think it pertinent to utilize a structured screening tool (PHQ-9, GAD-7, etc) with Peter? Why or why not?
You and Peter worked through the The Three T’s (Problem Context) of his concerns (stomach ache, headache, poor sleep ), what does Peter define as his problem? What goal do you all decide to focus on and why?
