State your practice question in PICOT format. (MY PICOT- In adult patients diagnosed with type 2 diabetes at a primary care clinic, does implementation of the CDC Diabetes Prevention Program, compared to current practice, impact fasting blood glucose, over 8-10 weeks?)
State the title of your Project: The title should inform and engage the reader and must be succinct. The title should include the intervention and the anticipated outcome and may include the population. (Please do not include abbreviations in the title.) Examine the problem at the practicum site and evaluate the gap in practice you will address with this project. To examine the problem at the practicum site and evaluate the gap in practice, you would first have to do the following:Identify the Problem: Begin by clearly identifying the problem or issue at the practicum site. This could be related to inefficiencies, gaps in service delivery, outdated processes, or any other challenge that hinders the optimal functioning of the organization (Bemker, Ralyea, Schreiner, 2021).
Conduct a Needs Assessment: Perform a needs assessment to gather information on the current state of affairs. This may involve interviews, surveys, observations, and a review of relevant documents (Bemker, Ralyea, Schreiner, 2021). Understand the perspectives of different stakeholders to get a comprehensive view.
Analyze Current Practices: Examine the existing practices and processes that contribute to or exacerbate the identified problem (Bemker, Ralyea, Schreiner, 2021). Look for patterns, root causes, and any systemic issues that may be contributing to the gap in practice.
