APA FORMAT TITLE & REFERENCE PAGE DOUBLE SPACED 12pt. FONT 6 PAGES For Part 1 of this project you will watch the documentary, Kind Hearted Woman (5 hrs), taking as many notes as you deem necessary to highlight the facts, key problems, and strengths of the all identified family members.
You will be working to identify strengths and challenges for each individual from a biopsychosocial perspective as well has unique strengths and challenges they have as a family. Your notes should be organized in a way the makes sense to you and are understandable to anyone who may need to review your work. Examples may include organizing by individual and family, organizing by identified issue (physical, psychological, medical, educational, work, criminal, etc.) Your notes are the foundation of any case management service you provide so it is important to be as thorough as possible. There may be items that seem small but could prove essential to the overall services being provided to the family so be as detailed as possible. Using your notes and the attached case management assessment tools, we will: PART 2: 1. Identify key problems, why they exist, and how they impact the individual or family. 2. Explore possible solutions or changes that are needed for each individual. Students can use course readings, discussions, their knowledge of local service resources, or conduct an internet search of services that have been used for the identified problems. Live sessions will be held during the week for students to ask questions, get some direction, and share ideas with each other on how they would identify resources. Case management is not meant to be a solo activity, use your classmates! 3. Select a ″best″ solution for each individual′s problem. Students will be expected to provide supporting evidence, pros, cons, and determine if the solution is realistic for the individual′s current needs. The solution should also include any additional requirements to implement (referrals to other agencies, transportation, funding availability, etc.) and what the expected outcome is (what does success look like?). This is an opportunity to be really creative and think outside the box (cliché′, I know)! Some of the best case managers are those who can create solutions that maybe haven′t been tried or even thought of before. Don′t be afraid to put forth an idea that could benefit the family. This is also why focusing on a family′s strengths can be really useful. Strengths can become their own solutions if we use them in a beneficial way Part 3: Is combining Part 1 & 2 together (Assessment Part) PART 4: The final step of the case study is to use your notes, assessment, and goals plan to build the case study analysis. The following template will be used to create your final report. I. Introduction (Just a Quick Overview of what I am about to read) – Identify the key individuals in the case and the primary individuals who are the target clients. – Identify the key problems and issues in the case study – This is an opportunity to introduce your family, the issues that you will be addressing in working with them, and proposed outcomes. – This section will be a snapshot of what you will be presenting in the body of the document II. Background – Set the scene: background information, relevant facts, and the most important issues. – Demonstrate that you researched the problems in the case study III. Evaluation of the Case – Outline the various pieces of the case study that you are focusing on. – Evaluate these pieces by discussing what is working and what is not working (strengths and concerns) – State why these parts of the case study are or are not working well. – Is there information missing from the assessment that might impact the work you do with the individuals or family? IV. Proposed Solutions/Changes – Provide specific and realistic solutions(s) or changes needed – Explain why this solution was chosen – Support this solution with solid evidence, research, such as: – Concepts from class (text readings, discussions, lectures) – Outside research – you will need to document your sources to avoid plagiarism. – Personal experience and knowledge V. Recommendations – Determine and discuss specific strategies for accomplishing the proposed solution – If applicable, recommend further action to resolve some of the issues – What should be done and should do it? Your analysis will be written in APA format, double-spaced, 12 pt font. There is an APA template located in Microsoft Word that I will cover in a live session as well as how to properly reference your source material. The analysis will be no less than five pages, not including the title and reference pages.
