The purpose of the clinical log, for DFC integration, is an accountability check and self-report of time spent in a DFC at a SOFE site.
Instructions: Download the clinical log form to track the DFC hours/time as a running list. Submit in the appropriate location (below), within 24 hours of the end of each DFC.
